
JOB TITLE: Activity Director
FLSA STATUS: Non-Exempt
DIVISION: Social Services
RATE: $18.00 - $26.22 per hour
Status: Full-Time
Schedule: Monday - Friday, 8:00 a.m. - 4:30 p.m. Occasional weekend and evening hours as needed.
POSITION SUMMARY:
Under direct supervision of the social services manager, responsible for originating, scheduling, supervising and evaluating supporting activities program for long term care residents and swing bed patients.
ESSENTIAL JOB FUNCTIONS:
1. Confer with the Social Services manager analyzing individual needs and limitations in planning activities that are incorporated in the overall resident plan of care.

2. Keep records of residents’ attendance, participation and attitude on quarterly basis and with significant changes.

3. Procure and maintain supplies and equipment in a safe manner, maintaining the activity area in good order.
​
4. Find sources of salvageable and donated materials and use them to the fullest extent possible in order to maintain costs.
​
5. Secure outside entertainment and confirm their scheduled performance.
​
6. Invite and encourage residents to participate in each day’s activities where appropriate, finding methods to motivate them to take an interest in life and the community.
​
7. Responsible for spiritual care of residents, making Chaplain’s list available to residents and scheduling regular services in the facility.
​
8. Demonstrate positive attitude of customer service.
​
9. Cover for Social Services Designee as needed.
​
10. Complete assigned sections of MDS as requested (assessments quarterly and with significant changes).
​
11. Attend Care Plan meetings and Risk Management meetings.
​
12. Develop monthly activity calendars.
​
QUALIFICATIONS:

Must have valid CNA license.

One year experience working in resident activity program or the equivalent in a health care setting or the enrollment in an activities director course within 120 days after employment.

Knowledge of philosophy of rehabilitation.

Must have acceptable KBI record check.

Must be 21 years of age or older.

Must have valid driver's license and ability to drive company vehicles as directed.
SKILLS:
Creative capabilities, flexibility, patience, ability to understand recreational needs of long term care individuals and possess an out-going, pleasant manner and sincere desire in working with them. Candidates must have exceptional time management skills.
EQUIPMENT TO BE USED:
Computer, telephone, fax machine, and CNA-related equipment.

SUPERVISORY REQUIREMENTS:
Oversees volunteers who are assisting with activities.
TYPICAL PHYSICAL/ MENTAL DEMANDS:
Regularly required to sit, talk or hear. Frequently required to sit for long periods of time, stand, use hands, reach and walk, stoop and bend; must occasionally lift or move with appropriate body mechanics up to 30 pounds. Must be able to perform tasks with frequent interruptions
WORKING CONDITIONS:
The employee works in an environment with biological, chemical and mechanical hazards. The employer will provide appropriate training, protective equipment and engineering controls for the protection and safety of the employee. As a condition of employment the employee is required to abide by all risk management regulations including the proper use of protective devices and procedures.
